Keto pills and drops: what's actually inside them and what the research shows

Keto pills promise the results of a ketogenic diet without the diet: drop a capsule or a dropperful under your tongue and your body starts burning fat. The label rarely says which of three very different things is inside. Some products contain actual ketones. Some contain raspberry ketone, which has nothing to do with ketosis. Most contain a long list of herbs at doses nobody prints. This guide sorts out which is which, what the research shows for each, and what to check before you spend money on a bottle.

What Keto Pills Actually Contain

On a ketogenic diet, cutting carbohydrates low enough makes your liver turn fat into ketone bodies, mainly beta-hydroxybutyrate (BHB). Your body runs on those ketones instead of glucose. That state is ketosis, and it comes from what you eat, not from what you swallow alongside it.

Products sold as keto pills and drops usually fall into one of three groups:

  • Exogenous ketones. BHB salts (BHB bound to sodium, potassium, calcium or magnesium) or ketone esters. These really do raise blood ketones for a few hours.
  • Raspberry ketone. A flavor compound from raspberries. Chemically it is a ketone, but it is not a ketone body and it doesn't put you into ketosis.
  • "Keto support" blends. Garcinia cambogia, African mango, green tea extract, caffeine sources and amino acids, often in a proprietary blend. "Keto" here is a marketing word, not a mechanism.

Each group has a different evidence base, so they get separate sections below.

Exogenous Ketones: Real Ketosis, Short-Lived

The exogenous ketone research is legitimate, but it uses doses measured in grams, delivered as drinks. In a 2017 Oxford study in Frontiers in Physiology, volunteers drank roughly 12 or 24 grams of BHB as a ketone ester or as ketone salts. Both raised blood ketones, and both wore off within three to four hours. The salt drinks raised D-BHB, the form your body makes, far less than the ester did (1.0 vs 2.8 mM at peak). That's partly because half of the salt product was L-BHB, a mirror-image form that lingered in the blood for more than eight hours.

A 2023 meta-analysis in Complementary Therapies in Clinical Practice pooled 30 randomized studies with 408 participants. It confirmed that ketone supplements raise blood BHB reliably. But nearly all of that research measures what happens over a few hours. It does not measure fat loss over months.

When researchers did test fat loss, the supplement didn't add anything. In a 2023 trial in Frontiers in Physiology, 32 female recreational athletes cut calories by 30% for four weeks. One group added 20 grams of ketone ester three times a day, another added extra protein, and a third took a placebo. Fat loss was the same in all three groups, about 2.6 kg on average. The authors cite martial arts as one of the sports where athletes have to make weight. Ketones didn't change body composition. The protein group was the one that kept its muscle.

There is one appetite signal worth knowing. In a small 2018 study in Obesity, a ketone ester drink lowered the hunger hormone ghrelin and reported hunger for a few hours, compared with a sugar drink with the same calories. That's interesting, but it was one dose in 15 normal-weight people, not a weight-loss trial.

Raspberry Ketone and Garcinia: Weak Evidence

Raspberry ketone shows up in more keto pills than any real ketone does. A 2015 review from the Technical University of Denmark in Regulatory Toxicology and Pharmacology found supplement doses of 100 to 1,400 mg a day, while raspberries contain at most 4.3 mg per kilogram of fruit. The toxicology data was limited to short-term rat studies, and the authors called the doses on the market a safety concern until better data exists. The human trials we found tested it only inside multi-ingredient blends, so nobody can say what raspberry ketone does on its own.

Garcinia cambogia has more research, and the results are modest. A 2011 meta-analysis in the Journal of Obesity pooled nine trials of its active compound, hydroxycitric acid (HCA), at 1 to 2.8 grams a day. The average extra weight loss over placebo was 0.88 kg, which the authors described as small, with uncertain clinical relevance. A larger 2020 review in Diabetes, Obesity and Metabolism covered 54 placebo-controlled trials of herbal weight-loss products. It found no effect for garcinia and concluded that the evidence wasn't strong enough to recommend any of the herbs it reviewed. The National Center for Complementary and Integrative Health (NCCIH) adds a safety note: there have been numerous reports of liver toxicity in people taking garcinia cambogia products.

African mango (Irvingia gabonensis) is the third common ingredient. A 2013 systematic review in the Journal of Dietary Supplements found three positive trials, all with flawed reporting, and concluded that its effect on body weight is unproven.

How to Read Keto Pills and Drops Labels

Turn the bottle over and look for three things.

  • Is there any BHB, and how much? If the product claims to raise ketones, look for beta-hydroxybutyrate or a ketone ester with a gram amount. The studies above used 12 grams or more per dose. A capsule holding a few hundred milligrams is nowhere near that.
  • Is it a proprietary blend? Blends must list ingredients by weight, from most to least, but they don't have to print individual amounts. Divide the blend weight by an ingredient's position and you get a ceiling: the fifth ingredient in a 500 mg blend can't be more than 100 mg.
  • What are the stimulants? Guarana, green tea extract and caffeine add up. If you already drink coffee or take a fat burner supplement, count every source.
GMA WARRIOR PICK

GMA Keto Max Drops is our liquid support formula for people already training and eating with intention. Straight talk: it contains no BHB or ketone ester, so it won't put you into ketosis. That part is your diet's job. Each 30-drop serving (10 drops, three times a day, held under the tongue) holds a 200 mg proprietary formula of 17 ingredients, led by raspberry ketone, African mango and L-carnitine. The label lists them by weight, so raspberry ketone is at most 200 mg and garcinia, 15th on the list, is at most about 13 mg. That is far below the 1 to 2.8 grams of HCA used in the garcinia trials. The blend also includes guarana (a caffeine source) and licorice extract. If you take any medication, have a liver condition, or are pregnant or nursing, check with your doctor or pharmacist first. Follow the label.

What Actually Moves the Needle

The trials above point the same way. Fat loss followed the calorie deficit, whether or not people took a supplement. A ketogenic diet can help some people get there because it cuts out easy-to-overeat foods and many people find it filling. The pill doesn't add to that.

  • Eat enough protein. In the 2023 athlete trial, extra protein was the only addition that kept muscle during the cut. Our guide to how much protein you need covers the numbers.
  • Keep training hard. Muscle you keep is muscle that keeps burning energy.
  • Be suspicious of shortcuts. Ads with celebrity endorsements and "no diet, no exercise" promises are a red flag. The same goes for "detox" claims, which we cover in our look at detox supplements.
  • Talk to your doctor first if you take any medication, have a medical condition, or are pregnant or nursing. A strict ketogenic diet changes how some medications behave, and that's a conversation to have before you start.

In more than fifty years of running a martial arts school, we've watched plenty of fighters try to diet their way down a weight class. The ones who succeeded weren't the ones with the best supplements. They planned their meals, slept, and kept showing up to train on a consistent schedule. Supplements can support that routine. They don't replace it. For more on what the evidence says about weight-loss products, read our guide to apple cider vinegar for weight loss, or browse the full Weight Loss & Detox collection.

Questions We Get

Frequently Asked Questions

Do keto pills actually work?

Not for fat loss on their own. Ketone supplements do raise blood ketones for a few hours, but in a 2023 trial of athletes on a calorie-restricted diet, adding 60 grams a day of ketone ester didn't change fat loss compared with placebo. Herbal "keto" ingredients like garcinia cambogia show small or no effects in reviews. Fat loss follows the calorie deficit. Any supplement is a small add-on to diet and training, not a replacement.

Do keto pills put you in ketosis?

Only products that contain real ketones (BHB salts or ketone esters) raise blood ketones, and the research doses are around 12 to 24 grams, usually as a drink. The effect fades within about three to four hours. Products built on raspberry ketone, garcinia or other herbs don't contain ketone bodies and won't put you into ketosis. Nutritional ketosis comes from cutting carbohydrates, not from a capsule.

Are keto pills safe?

It depends on what's in them. NCCIH notes numerous reports of liver toxicity in people taking garcinia cambogia products, and a 2015 review flagged raspberry ketone doses on the market as a safety concern because toxicity data is limited to rat studies. Many blends also contain stimulants. Read the full label, and talk to your doctor or pharmacist first if you take medication, have a liver condition or are pregnant or nursing.

What is the difference between keto pills and keto drops?

Mostly the format. Drops are a liquid you hold under your tongue, and pills are capsules or tablets. The ingredients are often similar: raspberry ketone, garcinia, African mango, amino acids and caffeine sources. Neither format turns a weak ingredient into a strong one. Check the supplement facts panel for the total blend weight and where each ingredient sits on the list. That tells you more than whether it's a drop or a pill.

Can you take keto pills without doing the keto diet?

You can, but you shouldn't expect keto-diet results. The ketogenic diet works for some people because cutting carbohydrates changes what and how much they eat. Supplements don't replicate that. Even real ketone drinks only raise ketones temporarily, and the one fat-loss trial we cite found no added benefit. If you want results, focus on a sustainable calorie deficit, enough protein and consistent training, with your doctor's input if you have a medical condition.
